As I have also shown that at term sheep pregnancy weighing 40Kg (duration of sheep pregnancy is around 140 days) the volume in the feto-placental circulation reaches to 600ml/min. The placental circulation occupies the 40-50% of this circulation and the feto-placental circulation is regulated mainly by small peptide hormones. The fetus produce angiotensin from beginning and throughout of gestation and acts as a hormone regulating blood circulation between the fetus and the placenta similar to vasopressin. Both angiotensin and vasopressin levels in the feto-placental circulation are known to increase in case of fetal hypoxia due to preeclampsia and difficult delivery. (Ref1) Small peptide hormones are active and have an essential role not only in the feto-placental unit, but also human body. The concentrations of these hormones acting in the human body are called to be the name of physiological levels. When the various parts of human placenta are exposed to the physiological concentration of both angiotensin and vasopressin, vessels of the various parts of human placenta are actively contracted. Namely both angiotensin and vasopressin are the actively contractile hormones of the placenta. (Ref. 2) When the pregnant sheep is exposed to low oxygen air, it is known that the fetus change its blood flow distribution. When the fetus is exposed to low oxygen air, the fetus increases the blood flow (distribution) in the vital organs such as placenta and brain. This change of the fetus in response to stress such as hypoxia is called to be the redistribution of the blood flow in the vital organs. (Ref.3) Let’s arrange our thinking as follows: 1.fetal blood pressure is mainly regulated by placenta circulation. 2. Both angiotensin and vasopressin contact actively placental vessels. 3. The fetus under the hypoxia increases both angiotensin and vasopressin. While the fetus exposed to hypoxia redistributes its blood flow for surviving, the fetus increase both angiotensin and vasopressin. It looks like some contradiction, since both angiotensin and vasopressin contract actively the vessels of the placenta. Next let’s resolve this contradictive question. Ref. 1 Kingdom JCP et al. As I have described in this lecture No.3 and 5, small peptide hormones such as vasopressin and angiotensin play an important role in the feto-placental circulation.

調査によると、2005―2012年の総分娩数は518024件で、子癇が203件(総分娩数の0.04%)、脳卒中が51件(総分娩数の0.01%)発症しています。 子癇203件の発症時期は、妊娠中と分娩中が56%を占めています。子癇が起きた施設は、大学・周産期医療センタ―が36%、総合病院が22%、一次医療施設が38%となっています。 さて、この統計結果をどう理解すべきでしょうか? 一次医療施設とは、産婦人科の開業医です。子癇が起きた施設は、開業医と高度周産期医療を担う大学や周産期医療センタ―とで、差がないのです。子癇や脳卒中は、妊娠高血圧症候群(妊娠中毒症)の極限です。 妊娠高血圧症候群は、もちろん、胎盤機能低下などによる胎児well-being(胎児が健全か否か)の悪化があります。 以前のブログでも書いたように、産婦人科診療ガイドラインは、胎児well-being の評価法として次のような方法を列挙しています。 1、 NST,CST 2、BPP(Biophysical score ) 3、推定体重の推移 4、羊水量の測定(超音波法) 5、超音波パルスDoppler法による臍帯動脈血流-などです。これらの専門用語に関しては6月16日のブログでご説明致しました。 開業医と比べ、高度周産期医療施設では当然、1-5の方法で妊娠高血圧症候群の妊婦の胎盤機能低下を詳しく検査しているはずです。 しかしながら、愛知県の調査結果では子癇の予知が高度周産期医療施設でも出来ていないことになります。 現在、高度周産期医療施設で実施されている、超音波やMEによる妊婦管理法に限界を感じているのは私だけなのでしょうか? 皆さんも、改めて妊婦管理の在り方はどうあるべきかを一緒に考えてみませんか。
愛知県周産期医療協議会調査研究事業「愛知県全分娩施設における子癇、妊産婦脳卒中、妊産婦血圧管理に関する研究」という表題の冊子が、手元にあります。
厚労省やPMDA(通称パンダ、独立行政法人医薬品医療機器総合機構)が7月、マグセントやマグネゾールを妊婦に使用する際の注意事項として、新生児の心機能障害を新たに追加したのは画期的で大きな意味があります。 まず、厚労省は今まで海外、特に米国FDAの薬剤副作用情報にほとんど追随してきました。しかし、新生児の心機能障害への注意喚起は、FDAは未だ行っていません。次に過去の記事で指摘してきたように、1歳以下で発症した心筋症例は圧倒的に拡張型が多いのです(日本心臓財団調べ)。 なぜ、生まれて間もない赤ちゃんが拡張型心筋症、つまり生まれる前から心臓の筋肉が発達しない病気になるのでしょうか? 厚労省は、1歳以下で発症する拡張型心筋症の赤ちゃんを産んだ母親の妊娠中にどんな薬剤が使われたかを早急に調査すべきです。 なぜなら、妊娠中毒症(妊娠高血圧症候群)や切迫早産治療のため、ヒトに使用される量と同量のマグネシウム剤を投与したラット胎仔の心筋組織の血管が形成されない実験結果から、心筋が薄くなる拡張型心筋症のリスクが示唆されるのです。(文献) この話は、以前にも記事にしました。市井の産科医ではリスクを指摘できても、妊婦にどんな薬剤が使われたかを調べることまでは出来ません。妊婦にマグセントやマグネゾールを使う場合、注意事項として「新生児の心機能障害」と明記した以上、厚労省には因果関係の調査をお願いしたいのです。 文献:Ishii M et al. 2013年5月30日、米国のFDA(日本の厚生労働省に相当)は切迫早産の治療に、硫酸マグネシウム(マグセントなど)を注射するのは5-7日までと勧告しました。
その理由は、妊婦への硫酸マグネシウムの長期投与は、胎児の骨の発育を傷害して(妨げて?)、胎児・新生児が骨量の減少、骨折などを引き起こす危険性があるためとしています。
このブログの過去の記事(2012年5月11日こちら,5月18日,6月11日,6月28日,10月21日,10月22日など)で、硫酸マグネシウムを妊婦に注射すると、胎児への悪影響が心配されると再三指摘してきました。
As previously mentioned the feto-placental circulation is independent of maternal circulation, the blood of both circulation is essentially not mixed with each other. This time I will explain the regulating factors of feto-placental circulation. As you might imagine, the research on human feto-placental circulation is difficult. Therefore the research is mainly based on the results of pregnant sheep and they are applied to human feto-placental circulation. Therefore the following evidences are the results of pregnant sheep. Mean blood pressure is calculated from one third of subtract diastolic blood pressure from systolic blood pressure. While mean blood pressure in adult is around 95mmHg, that in the fetus is around 45-50mmHg. The peripheral region of umbilical artery in the placenta changes into capillary and the blood pressure in the capillary is around 10mmHg. In the region of this capillary fetus takes oxygen from maternal circulation in the retro-placental blood pool and excretes wastes into maternal circulation. The blood supplied with oxygen in the capillary in the placenta flow out into umbilical vein and return to the fetus. The diameter of capillary is only 5-7 micron (1 micron is the one thousand millimeter). Since the number of capillary is too much, the total cross section is reached to 700 hundred hold than artery. The blood volume in the feto-placental unit increases with advancing gestation. The placental circulation occupies the 40-50% of this circulation. Therefore the placental circulation effects largely on the fetal blood pressure. In my previous lecture I have described the 3 main whole body factors, which regulate blood pressure: (1) hormonal and (2) nervous systems and local factor is (3) autonomous system. Let us show you the involvement of the 3 main factors in the feto-placental unit. Firstly autonomous system is shown: the change of blood flow in the aorta and placenta by ligation of fetal big artery, which locates in the inferior aorta of the fetus, was examined in relation to the degree of ligation. The ligation of aorta results in the increase of blood pressure. The placental blood flow decreased in parallel with the degree of ligation of aorta. This evidence show clearly autonomous system is not involved in in the feto-placental circulation. (Ref.1) The brain circulation is the example of autonomous system control of blood pressure. In the brain the change of blood pressure between 60-140 mmHg, does not effects on blood flow in the brain and blood flow does not change in the range of the above blood pressure. Next hormonal and nervous systems are described in my previous blog. Our visceral organs including vessels are influenced under the autonomic nerve systems. While adrenaline is released from sympathetic nerves and contract vessels, acethylcholine is released from parasympathetic nerves by stimulation of each nerves. Angiotensin and vasopressin are very active hormones for contraction of vessels. When norepinephrine (similar to adrenaline) and angiotensin were administered into sheep fetus, the change of blood flow and resistance (blood pressure) in the umbilicus were examined. In this experiment while norepinephrine does not effect on both of them, angiotensin decreases blood flow and increases resistance (blood pressure) in the umbilicus. These experiments show that feto-placental circulation is regulated by angiotensin and other peptide hormones (described in my previous blog), but by not nervous systems and autonomous system. Ref. ここ10年近く、WHO(世界保健機構)や厚生労働省は、活発な母乳推進運動を展開してきました。その効果は予想以上です。母乳第一主義の風潮は、すっかり日本社会に定着しています。
しかし、これは一面では“母乳信仰”ともなり、行き過ぎる傾向も招いています。
母乳の出ない母親は怠慢で母親失格とか、あるいは逆に人工ミルクは毒物という間違った考えも生まれました。
当然、人工栄養にせざるをえない母親は強いコンプレックスと子供の健康について強い不安感を持たされました。
言うまでもなく、近年のミルクはきわめてすぐれた製品です。これを用いることに何の心配も要りません。
母乳と人工乳の一番の違いは、分娩後1週間以内の母乳(昔は、鬼乳といいました。大変濃い色の出始めの母乳です)に少量含まれる分泌型免疫グロブリンA(IgA)、ラクトフェリンをはじめとするさまざまな感染防御因子が含まれていることです。
IgAは、各種の病原性微生物や毒素と結合して新生児を守る働きをします。分娩後1週間は、この鬼乳を必ず赤ちゃんに与えましょう。
1972年以来、母乳と人工乳で育てられた子供の発育に関して既に11回の詳しい調査が行われ、人工栄養児の体重と身長は、母乳栄養児と全く差がなく、人工栄養児の発育は母乳栄養児と同じで良好なことが明らかになっています。(文献)
分娩1週間後の母乳は、ミルクとなんら差がないのです。
8月1日は「世界母乳の日」、そして8月の第1週は「世界母乳週間」です。しかし過度な母乳推進で「母乳でなければだめ」「粉ミルクを与えるのは良くない」といった精神的なプレシャーを感じる必要はさらさらありません。
仕事と育児の両立のために積極的に人工ミルクを活用しましょう。
