Dr.水谷のお産講座No 44 妊婦が解熱鎮痛剤を服用するのは「安全」と言い切れるでしょうか。やむを得ず服用しても少量にしてください。


201411月に開かれた厚労省の薬事・食品衛生審議会の医薬品等安全対策部会で、市販の解熱鎮痛薬では人気ナンバーワンの「ロキソニンS」の販売規制をどう扱うかの話し合いがありました。この話し合いは、一つは妊婦が簡単に処方箋なしで、薬局で買えるこのヒット商品の胎児への副作用を厚労省が心配したからです。

▽市販されている一般的な解熱鎮痛薬は、いずれもプロスタグランジン(PG)という生理活性物質(微量で生体の生理作用を発現させる物質)が増えるのを抑えることで痛みや発熱による炎症を抑えます。痛みや発熱などは、全てプロスタグランジンが増えることによるのです。痛みが和らげられるため、妊婦がつい手を伸ばしたくなる気持ちは分からないではありません。

▽プロスタグランジンを抑えるロキソニンなどの薬剤を、妊婦が服用すると、それらの薬剤は胎盤を通過して胎児に移行した場合,胎児の動脈管が閉じて(収縮して),胎児循環持続症(PFC)と呼ばれる病気になる危険性が指摘されています。

▽なぜなら、PGは胎児の動脈管が開いている(拡張)状態を維持するのに最重要な物質だからです。そこにPGを少なくする作用のあるロキソニンやアスピリンなどの解熱鎮痛薬を服用したらどうなるか、お分かりでしょう。

▽前後しましたが、動脈管を簡単に説明します。動脈管は、胎児が妊婦のお腹の中にいる時、肺動脈から大動脈への抜け道になっている血管です。胎児は酸素を胎盤から(母体)もらうため、肺呼吸をしていません。だから胎児肺への血液の流れは必要ないのです。


▽出生後、赤ちゃんが肺で呼吸を始めると、動脈管は不要になり、生後23週までに完全に閉じてしまいます。動脈管が自然に閉じずに残っている症状を動脈管開存症といい、最も多い先天性心疾患のひとつです。出生例の510%を占めています。

▽胎児の動脈管は、合成されるPGの作用で開いています。このためロキソニンなどPGを抑える薬剤は、胎児の動脈管を閉じて(収縮して)しまいます。胎児の動脈管が閉じと、生まれてきた赤ちゃんの肺動脈の血圧が上がります。


▽すなわち、肺呼吸をしていない胎児の循環動態と同じになります。この状態(新生児肺高血圧)が、以前は胎児循環持続症(Persistent fetal irculation、PFC)と呼ばれていました。

▽アスピリンもロキソニンと同様にPGを抑えて解熱鎮痛作用をあらわします。米国FDA(日本の厚労省に当たります)は、妊婦のアスピリン使用量が多いと、新生児の死亡、子宮内発育不全、あるいは胎児奇形になると警告。妊娠中のアスピリン投与は、明確に有益性がその危険性を上回る時のみに使用が許されるとしています。

▽さらにFDAは、分娩前1週間までアスピリンが投与されている妊婦は、明らかに分娩時の出血が増加すると警告しています。また分娩が長引いたり、分娩予定日を過ぎたりしても、陣痛が来ないケースが多いとしています。

▽一方、アスピリンは、妊娠中毒症(妊娠高血圧症)や不育症、流産などの予防に効果があると考えられており、このような疾患にも使用されています。しかし、アスピリンもPGの作用を抑えることから、ロキソニンと同様、胎児への副作用が心配されることは、ご理解いただけると思います。

▽英国の医学雑誌『Lancet(ランセット)』は、過去に流産を経験している1078人の女性を対象に、妊娠前に投与開始した低用量アスピリンの妊娠転帰への影響を調べた無作為比試験(EAGeR試験)の評価を掲載しています。それによると、アスピリン服用群とプラセボ服用群の生児出生率は58%と53%(P=0.0984)、流産率は13%と12%(P=0.7812)。低用量アスピリンと妊娠転帰に有意な関連はないことが示唆されたとしています。

▽言い換えると、アスピリンには流産予防の効果がないことを物語っています。にもかかわらず、アスピリンを妊婦に安易に使用されているのです。

Lecture on blood pressure in pregnancy No.11: The new concept of renin-angiotensin system in Feto-placental unit: a growing local (tissue) renin-angiotensin system (RAS)

 
 Renin was the name given in 1898 by Tigerstedt and Bergman to the pressor material in extracts of rabbit kidneys. Renin is secreted by the juxtaglomerular cells located in the wall of the afferent arteriole, which is contiguous with the macula densa portion of the same nephron. In humans renin is present in kidney and plasma. Renin acts as a proteolytic enzyme to split off of a portion of the
α- globulin known as renin-substrate or angiotensinogen. The portion split off by renin is angiotensin I (A- I) and contains 10 amino acids. As it courses through the circulation, the two end amino acids histidyl and leucine are removed by angiotensin-converting enzyme (ACE), forming the eight amino acid angiotensin A-II. Conversion occurs throughout the body, but the highest levels of the ACE are found in the lung. A-II exerts the primary physiological effects of renin-angiotensin system (RAS). This is the standard concept of RAS. Later the concept of tissue RAS system has appeared from the research of brain. Recently many studies have shown that there is a RAS in each organ such as the kidneys, brain, and testis, which may be essential to regulate the local and systemic circulations. Local angiotensin -II (A -II) production process is commonly termed the result of a” local” RAS.


Since feto-placental unit produces A-II with advancing pregnancy, it is regarded to be one of the local RAS for pregnant women. The human fetus produces renin very early at 5-6 weeks of gestation. Thus, the granular cells of the fetal kidney are one of the earliest endocrine cells detectable during human ontogenesis. Renin specific activity in the fetal kidneys is about 20 folds greater than that of normal adult kidneys. This high activity suggests that the fetal RAS is chronically stimulated and may be an important factor for controlling blood pressure in feto-placental circulation.

In the feto-placental circulation , while angiotensin I (A- I) levels in umbilical artery are significantly higher than umbilical vein, A-II levels in umbilical artery are significantly lower than umbilical vein, suggesting that the conversion from A- I to A-II occurred in the placenta. Considering that the fetal lung is inactive, the main site for the conversion of A- I to A-II by angiotensin converting enzyme (ACE) is in the placenta. Broughton Pipkin and Symonds measured a significant difference in A-II concentrations comparing umbilical venous and arterial blood during preeclampsia, suggesting increased A-II release from the stressed or transiently hypoxemic fetus. Kingdam et al. noted a relationship between umbilical arterial oxygen partial pressure and A-II levels in the umbilical vein during delivery, suggesting increases in A-II levels in parallel with fetal asphyxia.

These results indicate that A-II levels are elevated in the human fetus under stress such as hypoxia. Although the transfer of A-II across the placenta is negligible in normal pregnancy, and maternal A-II has little effect on the fetus, it is provable that increased A-II from stressed fetus may leak into maternal blood and may resulted in the contraction of vessels of pregnant women. In this regard, the important roles of aminopeptidases in the course of pregnancy is reassured. Maternal serum aminopeptidase A (APA), which is the responsible enzyme for degradation of A-II, activities in early mild preeclampsia patients are higher than those seen in normal pregnancy. While the severity of preeclampsia progresses and the APA activities decreases, the phenomenon suggesting physiological dynamics of cross function of fetal peptides and aminopeptidases in complicated pregnancy, that ultimately becoming lower than that measured during normal pregnancy. APA might be the essential substance for regulating the local RAS in other organs such as the kidneys. Until now there is no pointing out that feto-placental unit is the one of local RAS system in pregnant women.

 

Dr.水谷のお産講座 No. 43. NPOが提言する早産と妊娠高血圧症(妊娠中毒症)の新しい治療法(ホルモン療法)の論文がドイツの国際的医学誌に採用されました。






▽私たちのNPOは、妊娠高血圧症や切迫早産の治療で、胎児と妊婦さんに安全で優しい治療とは何かを真剣に考え続けています。それは、今なお、十分解決されていない大きな医学上の問題になっているからです。





▽ただ現行の標準的な治療方法を「母子に危険」と批判するばかりでは、あまりに無責任です。NPO理事長・水谷栄彦名古屋大名誉教授は、過去の臨床と膨大な基礎研究の成果を基に、代替治療法に関する論文をドイツ・ミュンヘンの雑誌社『Georg Thieme Verlag KG』に投稿し採用されました。





▽インターネットの時代です。この代替治療法(ホルモン療法)の存在は、世界中に広がっていくでしょう。論文のタイトルは「New Insights into the role of sex steroid hormones in pregnancy: possible therapeutic approach by sex steroid hormones for the treatment of both preeclampsia and preterm labor」です。

▽妊娠期間は胎児の成長とともに母体とのホルモン動態の恒常性を保つことが極めて重要になります。そして、妊娠末期でいよいよ出産する場面ではホルモンとそれらの分解物質との微妙なバランスが適切な分娩タイミングを決定する要素となります。





▽このバランスが破たんすると、妊娠高血圧症や切迫早産のように胎児や妊婦さん、あるいは双方の命とりになりかねません。この考え方を分かっていただけると、万が一、バランスが破たんをきたすような場合、それを修復するため、適切な量のホルモンを母体外から投与するホルモン療法が最も理に適っています。





▽ただ残念ながら、この基本的なメカニズムは、いまだに医学界でも十分周知されていないのが現状です。その結果、胎児、さらには母体にも危険な影響を与えかねない薬剤が漫然と使われています。薬剤の危険性は2014年11月18日発売の「週刊朝日」でも4ページの特集を組んで指摘しています。





▽さらに胎児が十分発育していないにもかかわらず、母体の帝王切開を選択して胎児を取り出す方法が採られています。





▽論文は、ステロイド治療(steroid hormones for the treatment)となっていますが、この場合のステロイドは女性ホルモン、即ちエストラジオール(女性ホルモン)とプロゲステロン(黄体ホルモン)です。
